In its first year, the CHS Cares Fund awarded approximately $1.5 million in grants to help nearly 700 colleagues in need.

About the CHS Cares Fund

Established in 2017, the CHS Cares Fund provides financial assistance to employees of Community Health Systems affiliated hospitals, clinics and offices who have experienced hardship due to events beyond their control such as natural disasters and extended illnesses or injuries. The Cares Fund’s grants can help pay for essential living expenses such as housing, utilities, food, clothing and other basic necessities.

All gifts to the CHS Cares Fund are tax deductible and will help individuals across the organization in their time of need.

How to Apply for Assistance

Step 1: Review the eligibility requirements by clicking HERE. If you qualify, proceed to step 2.

Step 2: Complete all sections of the CHS Cares Fund Application for Assistance. Using any connected device (phone, laptop, tablet, etc.) you can apply for help online HERE.

Step 3: Submit your application and documentation to the Fund’s administrator, The Community Foundation of Middle Tennessee

Step 4: Cares Fund staff will review your application and determine if you qualify. Once you have submitted your application and documentation to the Community Foundation of Middle Tennessee, your application will be reviewed for completeness. You may be contacted by Cares Fund staff if additional information or documentation is needed. If your application is approved, the Cares Fund staff will make payment(s) on your behalf to the vendor(s) you specify. All grants are made directly to vendors as bill payments; no assistance funds will be sent directly to you, and no reimbursements can be made.
